• You will become part of a supportive physician community.
• Each care home works closely with an interdisciplinary care team to provide holistic resident‑centered care.
• Opportunity to join the Long Term Care Initiative (LTCI) which includes paid mentorship, participation in a self‑scheduling paid after‑hours care system (one week coverage per quarter), paid participation in LTC‑specific events, quality improvement projects, and CME.
• You can develop skills in caring for the frail elderly and transfer them to community and hospital settings.
• Allows flexibility – supplement other family practice work with a LTC panel to add variety. Work with the care home leadership to set panel size and availability.
• Caring for the frail elderly is challenging and rewarding.

Remuneration: LFP model or fee‑for‑service with additional incentive payments.

Remuneration Details: LTCI care home panels receive payments for after‑hours care program – $1,000 for weekday shifts (5 pm‑8 am Monday–Thursday) and $1,400 for weekend shifts (5 pm Friday‑8 am Monday) – and sessional payments for mentorship activities, events, and quality improvement projects.
