Overview
Vancouver Coastal Health (VCH) is proud to be part of the provincial implementation of minimum Nurse to Patient Ratios (mNPRs), an initiative aimed at enhancing nursing practice and strengthening quality of care across the health system. mNPRs set the minimum number of nurses providing care to patients on a given unit. In British Columbia, mNPRs are developed in partnership with the Ministry of Health, the BC Nurses’ Union (BCNU) and health organizations.

The salary range for this position is CAD $41.42/Hr.

Complex Care Clinician – Sunshine Coast Home Care, Sechelt, BC
Responsibilities- Within a comprehensive care coordination model and within the context of a client and family centered care model, applies specialized theory, clinical expertise and knowledge relating to designated/specific client populations.
- Provides wrap-around care to designated client populations, with a strong commitment to client and family-centered care.
- Leverages clinical expertise to address complex medical, cognitive, and behavioral needs of eligible clients, primarily focusing on safe, ethical and high-quality care.
- Ensures that each client's care plan is tailored to their specific needs and goals, integrating prevention, chronic disease management, palliative care, and self-care management as needed.
- Performs ongoing assessments to identify issues or deviations from the care plan and prompts collaboration with medical providers, care providers, interdisciplinary team members, and community agencies.
- Collaborates with interdisciplinary team members, leadership, and other healthcare professionals to enhance quality of life and client/family-centered care.
Practices within the context of a client and family centered care model and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for registered nurses and registered psychiatric nurses. Aligns with organizational policy, guidelines and the VCH values, including Indigenous Cultural Safety, Anti-Racism, Equity, Diversity and Inclusion, and Planetary Health.
Education & Experience- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two years’ recent case management or home care nursing experience, including work with complex medically frail populations, or an equivalent combination of education, training and experience.
- Local area travel requires the use of personal vehicle. Valid BC Driver’s License.
- Knowledge of nursing practice related to Home Health Preventative Complex Care clients.
- Ability to perform comprehensive, evidence-informed and strength-based holistic health assessments and evaluate health promotion activities.
- Knowledge of assessment and screening tools (e.g., risk assessment tools, Inter-RAI, Zarit, MMSE) to support complex care decisions.
- Ability to develop client-centered care plans using evidence-informed practice.
- Critical thinking and clinical judgment to prioritize care during changes in acuity or emergencies.
- Engage clients and their circle of care as partners in planning and delivering care.
- Monitor and adjust care plans based on effectiveness and changing conditions.
- Promote shared decision-making, considering determinants of health.
- Ability to organize and facilitate meetings with clients and their circle of care to enhance client-centered care (e.g., case conferences).
- Competency in RN/RPN scope of practice and home health care standards (e.g., medication administration, wound care, vital signs, pain management, home support/ delegation).
- Knowledge of chronic disease management, palliative care, mental health and substance use, and self-management strategies.
- Skill in goals of care conversations and understanding client priorities.
