Vancouver Coastal Health (VCH) is proud to be part of the provincial implementation of minimum Nurse to Patient Ratios (mNPRs), an initiative aimed at enhancing nursing practice and strengthening quality of care across the health system. mNPRs set the minimum number of nurses providing care to patients on a given unit. In British Columbia, mNPRs are developed in partnership with the Ministry of Health, the BC Nurses’ Union (BCNU) and health organizations.

Salary Details

The salary range for this position is CAD $41.42/Hr.

- CAD $55.91/Hr.

TRAINING OPPORTUNITY

Candidates accepting this training opportunity agree to remain with Vancouver Coastal Health (VCH) in the area of nursing applicable to the education program for which they are hired for at least 18 months subsequent to completing training.
This will ensure that candidates are able to consolidate and apply skills in the specialty area that VCH has supported and funded candidates to complete. In the event that a candidate leaves the area of nursing applicable to the education program for which they were hired for VCH in under 18 months, VCH will seek to recover the cost of the program.

Within an interdisciplinary team–based care model and in collaboration with client/family/caregivers, Community Health Nurse cares for clients with acute, chronic and palliative care needs. Community Health Nurse works in partnership with the client and family to establish goals that are safe, realistic, and reasonable by assessing, planning, coordinating, organizing, implementing, evaluating, and transitioning nursing and other services for the client. Community Health Nurse assesses service eligibility, facilitates the development, implementation, and adaptation of a care plan, delivers direct care, and provides care management to assist clients to manage their own care and navigate through the various services available within the community settings.

Works in consultation with other system partners and incorporates current evidence into practice. Practices in accordance with established standards as outlined by the British Columbia College of Nurses and Midwives (BCCNM), Code of Ethics for Registered Nurses as well as within the VCH vision and values, policies, standards, and Decision Support Tools.

ABOUT HOME HEALTH PROGRAMS

Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.

THE CLIENT POPULATION

Home Health clients typically have on-going health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence.

Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • One year recent related clinical experience in a relevant clinical area working with complex clients/patients with chronic health conditions and/or functional mobility issues including recent experience in care and transition planning, or an equivalent combination of education, training and experience.
  • Valid BC Driver’s License required as local area travel may require the use of a vehicle or other accepted mode of transportation.
