RAI & Restorative Lead Coordinator – Full Time
Reporting to the Director of Care.
Responsibilities- Review interdisciplinary and comprehensive resident assessments, collecting data for quality documentation and ensuring adherence to MDS and Inter
RAI LTCF requirements. - Support nursing and accounting departments.
- Manage coordination and completion of Resident Assessment, Clinical Assessment Protocols and care plans as assigned.
- Comply with MLTC regulations regarding the Inter
RAI LTCF process. - Accurately code Inter
RAI and care plan documentation for all residents. - Continuously update knowledge base related to data entry and computer technology.
- Chair RAP chat as required.
- Provide restorative education, support and assistance to staff and residents.
- Assess new residents for restorative needs.
- Develop and document restorative goals and individualized plan of care for residents.
- Re-evaluate care plans per restorative care policy and procedure.
- Monitor and train staff regarding restorative tasks and resident‑specific goals.
- Record statistics and maintain records of required activities.
- Coordinate Bed Entrapment Prevention Program.
- Use supplies and equipment as directed and provide input on equipment selection.
- Attend care conferences as required and participate in Quality Management.
- Participate in the quality management program and complete restorative audits as required.
- Chair restorative committee.
- Participate in setting realistic and achievable departmental goals.
- Ensure residents’ care assessments, service plans and documentation are up to date and communicate changes to DOC for billing.
- Assist with scheduling residents’ care conferences and participate in conferences.
- Facilitate and provide resident/family education.
- Coordinate nursing team’s resident care assignments for efficient and timely service delivery.
- Respond to and manage significant changes in residents’ condition and medical emergencies.
- Address resident/family concerns and ensure appropriate action is taken and concerns communicated to DOC/Executive Director and other departments.
- Assist DOC in coordinating, implementing, and monitoring Infection Prevention & Control, Health & Safety, Clinical Programs and Quality Plans for the Nursing Department.
- 3-5 years experience in Long Term Care; experience with MDS is an asset.
- Current registration as an RPN in good standing with the College of Nurses.
- Current Inter
RAI certification and Relias certification. - Ability to maintain complete confidentiality of information.
- Knowledge and experience with adult education strategies/facilitation.
- Strong analytical skills and knowledge in trending and analyzing data.
- Well‑developed oral and written communication skills.
- Proficient computer skills and strong problem‑solving abilities with excellent time management.
- Clear aptitude for detail‑oriented work.
- Strong understanding of Point Click Care and RAI‑MDS.
- Excellent organizational, communication, and interpersonal skills.
- Flexibility to work variable shifts, self‑directed, and able to function in a fast‑paced environment.
- Vulnerable Persons Sector Police Check required (acceptable within 6 months).
- Flexible hours including days, evenings, nights, statutory holidays, and weekend shifts.
