Position: Community Health Nurse (RN)
Location: West Vancouver

Job Overview

Community Health Nursing with Vancouver Coastal Health (VCH) on the Home Health team at West Vancouver Community Centre provides care to clients with acute, chronic, palliative, or rehabilitative health needs. The role involves assessment, planning, coordination, implementation, evaluation, and transition of nursing and other services in partnership with clients, families, and interdisciplinary team members.

Salary

CAD $41.42–$55.91 per hour.

Responsibilities

The Community Health Nurse:

  • Assesses, plans, coordinates, organizes, implements, evaluates, and transitions nursing and other services for clients.
  • Determines service eligibility and facilitates development, implementation, and adaptation of care plans.
  • Delivers direct care and provides care management to help clients manage their own care and navigate community services.
  • Works in consultation with other system partners and incorporates current evidence into practice.
  • Documents patient status, interventions, and outcomes in the electronic medical record (EMR).
  • Operates within the standards of the British Columbia College of Nurses and Midwives (BCCNM) and VCH policies.
  • Supports the operational requirements of the collective agreement and applicable statutes, including flexible hours and days off.
Education & Experience
  • Current registration as a Registered Nurse with the BCCNM.
  • At least one year of recent clinical experience in a relevant area working with complex clients or patients with chronic health conditions and/or functional mobility issues, including experience in care and transition planning.
  • Valid British Columbia driver’s licence (local travel may be required).
Knowledge, Skills & Abilities
  • Knowledge and practice of community health nursing theory and client‑ and family‑centred care.
  • Trauma‑informed, harm‑reduction, and culturally safe clinical assessment and intervention skills.
  • Critical thinking to assess safety and risk in home environments and to problem‑solve using data from multiple sources.
  • Conflict resolution, reconciliation, and psychosocial problem‑solving related to goals of care, transitions, and end‑of‑life care.
  • Ability to manage and prioritize clients, promote cooperation, and pursue innovative practice.
  • Adaptability and resilience in responding to new or unexpected events while maintaining client‑focused care.
  • Team collaboration with interdisciplinary members and holistic assessment/plan/monitoring of health challenges.
  • Documentation skills using EMR, with timely and appropriate reporting of assessments, decisions, plans, interventions, and outcomes.
  • Proficiency with related computer software, applications, and devices.
  • Confidentiality in communicating, documenting, and handling client information across settings.
  • Effective communication—verbal, non‑verbal, written, electronic, and social media—with clients, families, and other health professionals.
  • Teaching, facilitation, and coaching skills employing adult education principles to support self‑management of care.
  • Knowledge of provincial acts, regulations, and program policies related to home and community care.
  • Understanding of acute, chronic, palliative, mental health, substance use, and health‑management issues.
  • Awareness of community resources for client/family support and related health services.
  • Familiarity with research, quality improvement, evaluation processes, and methodologies.
  • Organizational and scheduling skills.
  • CPR certification and use of nursing equipment and related techniques.
  • Physical ability to perform the duties of the position.
  • Broad knowledge of BCCNM standards of practice and clinical guidelines.
  • Ability to operate related equipment and computer applications.
