Supportive Case Manager in Home Health

As a Supportive Case Manager, you will work within a multidisciplinary team to manage a select population of Home Health clients. Key tasks include assessing client needs, determining goals, coordinating care plans, and providing continuity of care. Key Responsibilities:

• Assess and determine client goals and needs

• Develop and implement care plans with clients

• Liaise with interdisciplinary team members for client support

• Evaluate client outcomes based on established goals

• Provide education and clinical resources to team members

Requirements:

• Active RN registration with BCCNM

• Minimum of three years’ clinical nursing experience

• Valid BC Driver’s License for local travel

• Comprehensive knowledge of case management principles

• Ability to work independently and collaboratively
