Position: Nursing Role in Community Health Services
Join Vancouver Coastal Health as a Community Health Nurse and provide essential care in North Vancouver, BC. Your expertise will help clients navigate their health challenges while receiving support in their homes.

As a Community Health Nurse, you will work within a collaborative team to assess and address the acute and chronic care needs of clients. This role requires effective communication and problem-solving skills to develop and implement safe care plans. You will utilize your nursing knowledge to deliver compassionate care and assist clients in promoting their independence within the community.

Key Responsibilities:

• Assess client needs and develop tailored care interventions

• Coordinate services and facilitate client care transitions

• Provide education on health management to clients and families

• Maintain accurate documentation in the electronic medical record

• Collaborate with interdisciplinary teams to ensure quality care

Requirements:

• Current BCCNM registration as a Registered Nurse

• Minimum one year experience with complex client cases

• BC Driver's License for transportation needs

• Strong critical thinking and client-focused skills

• Ability to navigate community care resources
