Registered Nurse Coordinator

Transform health outcomes as a Registered Nurse Care Coordinator with ćećəwət leləm in North Vancouver. Provide culturally integrated nursing care focusing on wellness, independence, and client-centered support.

Reporting to the Health and Wellness Director, this full-time role emphasizes trauma-informed care in home and community settings. You will conduct comprehensive assessments, create personalized care plans, and implement evidence-informed interventions. Collaborate with clients, families, and interdisciplinary teams to foster independence and wellness through proactive care management.

Key Responsibilities:

• Conduct comprehensive assessments of client health status

• Create individualized care plans in collaboration with clients

• Implement evidence-based interventions to enhance client autonomy

• Manage case coordination to support client's ongoing care

• Navigate clients through available health resources and services

Requirements:

• Active Registered Nurse registration with BCCNM

• Bachelor’s Degree in Nursing from an accredited institution

• 3 to 5 years in home care nursing and case management

• Valid BC Driver’s License and reliable vehicle

• BLS certification and background screening required
