Key Responsibilities:

• Conduct thorough nursing assessments to guide care

• Implement resident-centered clinical interventions

• Collaborate with the interdisciplinary team effectively

• Support RCA’s and LPN’s in care coordination

• Engage families in care discussions and planning

Requirements:

• Registration as RN or RPN with BCCNM required

• Completion of gerontology course or equivalent training

• Comprehensive knowledge of aging-related medical conditions

• Demonstrated capability in chronic disease management

• Ability to manage end-of-life care discussions
