Vancouver Coastal Health is looking for a Community Health Nurse to join its Home Health Team. In this position, you will assess, plan, and implement nursing care for clients facing diverse health challenges, including chronic and palliative conditions.

Key Responsibilities:

• Complete initial and ongoing client health assessments

Requirements:

• Registered Nurse status with BCCNM is essential

• One year of relevant clinical experience required
