Position: Care Coordinator Registered Nurse Role

In this permanent, full-time position, you will report to the Health and Wellness Director. Your expertise in nursing assessment and case management will be crucial in developing tailored care plans that meet client needs. Engage in teamwork with families and professionals to monitor health transitions and ensure continuity of care throughout community settings.

Key Responsibilities:

• Promote self-advocacy and independence through therapeutic care

• Conduct environmental assessments identifying health barriers

• Facilitate communication and connection among care providers

• Design home support service plans for clients' daily activities

• Lead care conferences for coordinated client support

Requirements:

• Registration as a Registered Nurse with BCCNM

• Requires a Bachelor’s Degree in Nursing

• Minimum 3-5 years nursing experience with client care

• Class 5 BC Driver’s License and valid insurance

• Attendance in relevant training on Indigenous health preferred
