As a Registered Nurse Care Coordinator, you will report to the Health and Wellness Director, blending home care nursing with community health strategies.
Your role entails comprehensive assessments, tailored care planning, and coordination of services that uplift the health of the Tsleil-Waututh Nation. Ideal candidates utilize their nursing expertise to inspire enduring care relationships while advocating for client needs.
Key Responsibilities:
• Deliver trauma-informed home care assessments and interventions
• Tailor individualized health plans in collaboration with clients
• Foster care relationships with clients and families
• Assist in navigating health services and community resources
• Conduct ongoing evaluations to ensure optimal care delivery
Requirements:
• Current Registered Nurse registration with BCCNM
• Bachelor's Degree in Nursing from an accredited institution
• 3-5 years of home care nursing or case management experience
• Valid BC Driver’s License and reliable vehicle
• Basic Life Support (BLS) certification
