Community Outreach Nurse - Mental Health Focus

You will work within an interdisciplinary team, responsible for conducting thorough assessments and offering crisis intervention to clients dealing with mental health and substance use issues. This role emphasizes collaboration in care planning, ensuring continuity of care, and participation in client-centered practices.

Key Responsibilities:

• Conduct comprehensive assessments of clients' needs

• Provide effective crisis intervention and counseling

• Work collaboratively to develop integrated care plans

• Assist clients in accessing medical and community resources

• Engage in interdisciplinary case conferences

Requirements:

• Registered Nurse or Registered Psychiatric Nurse with BCCNM

• Two years’ experience with mental health issues

• Valid BC Driver’s License and vehicle access

• Knowledge of mental health recovery principles

• Ability to perform psychiatric assessments accurately
