Job SummaryAre you looking for a place to showcase your clinical hospital pharmaceutical skills? We have a Regular Full-Time opportunity for a Clinical Pharmacist to join our team at Lions Gate Hospital, located in North Vancouver, BC.
Responsibilities- Consult with physicians, nurses and other health care providers to develop and discuss therapeutic plans appropriate for each patient; ensure documentation is maintained in the patient health record.
- Individualize patient medication therapy by reviewing charts, lab results, and patient interactions; modify or develop therapeutic plans as needed.
- Provide patient medication counselling and educate patients and families on drug therapy.
- Assist in developing drug therapy guidelines for individual disease conditions; participate in medication use evaluations and quality improvement initiatives; contribute to clinical drug trials when applicable.
- Serve as department resource within the assigned area and mentor pharmacy students, residents and new staff.
- Validate prescriptions, resolve medication safety concerns, and document professional practice activities.
- Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
- Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience.
- Broad knowledge of organization policies, procedures and standards of care; knowledge of evidence-based clinical pharmacy practice.
- Knowledge of medication use evaluation and research processes; ability to apply clinical pharmacy knowledge with minimal supervision.
- Effective verbal and written communication; ability to establish and maintain working relationships with health practitioners, patients and families.
- Strong organizational and prioritization skills; ability to operate related equipment and manage dispensing processes.
