Position: Registered Nurse (Community Care Coordinator)

These Registered Nurse (Community Care Coordinator) positions are located in the Community Care Services at the Leeds, MA location. These positions are full-time at 40 hours per week.

Duties

Include but not limited to:

  • Responsible and accountable for all elements of the nursing process when providing and/or supervising patient care.
  • Assesses, plans, implements, and evaluates the effectiveness of the Veteran's care through telephone interaction.
  • Serves to support delivery of high quality, timely care to the Veteran receiving services in the community.
  • Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
  • Follows procedures per established policies and guidelines.
  • Influences care outcomes by collaborating with members of the interdisciplinary team.
  • Core elements of performance include knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.
Work Schedule
  • Monday - Friday, 8:00 am - 4:30 pm

Compensation & Benefits
  • Pay:
    Competitive salary, regular salary increases, potential for performance awards
  • Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

    Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA

Licensure
  • 1 full and unrestricted license from any US State or territory

  • These positions are in the Excepted Service.
  • Current and former Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held.
  • These positions are covered by a special rate.
