DESE/DDS Program Coordinator (Senior Case Manager)

Western Massachusetts Communities; main office in Northampton
Full-Time
Pay Range: $21.53 - $23.13/hour
Schedule:
Monday - Friday, 8 hours daily, between 9 a.m. – 5 p.m. with flexibility to meet case load needs; position is hybrid with some admin and meetings provided remotely, but ability to report to office when needed and in-person outreach supports are also required throughout Hampden, Hampshire, Franklin, and Berkshire Counties.
Service Net, a leading Human Services Agency, is seeking compassionate, dedicated, and enthusiastic individuals to join our team. The DESE/DDS Program partners with families to create and coordinate supports within the family home and is highly collaborative with other team members. The DESE/DDS Program Coordinator performs a variety of duties to support families with school‑age children, teenagers, and young adults with intellectual disabilities and/or autism by providing family supports, recruiting in‑home service providers, collaborating with larger team members, and co‑supervising a small team of Case Managers to provide services to families.
Key Responsibilities- Ensure proper supervision of staff to meet programmatic needs in collaboration with Director of Operations.
- Provide support to families: get to know families and children supported; make family visits; develop support plans/goals; provide information on resources and services; maintain contact and ongoing support as needed; connect families with other families for support; work with family and staff to promote community integration.
- Develop and track family contracts and budgets.
- Maintain accurate records for billing, data collection and family plans.
- Coordinate recruitment and credentialing of contracted service providers; participate in recruitment and support of providers.
- Maintain communication and coordinate with other agencies; serve as liaison with local DDS staff.
- Assist Directors in development of organization procedures and best practices to improve program efficiency and quality of services.
- Be able to work both independently and in close collaboration with team members when needed.
- Bachelor's degree preferred or education equivalent to a 4-year college degree.
- High School Diploma/GED required.
- Knowledge of autism and/or intellectual disabilities and support models.
- Experience with case & healthcare activities coordination.
- Over 3 years supporting adults and/or children with autism and intellectual disabilities.
- Family support experience a plus.
- Supervisory experience or clear readiness to supervise is a plus.
- Vehicle, valid driver's license and motor vehicle record required.
