Flow and Capacity team (FACT):
Undertake assessments; provide advice and co-ordinate support for people who are in hospital; discharged home with reablement services or who transfer from hospital to settings such as residential Discharge to Assess placements or the Recovery and Independence Bed Unit. Adult Social Care Workers play a vital role in supporting the safety, dignity and well-being of vulnerable adults. You will work with a wide range of individuals, including older people, adults with physical, sensory or cognitive impairments, and people with mental health needs, to include ASD.

The role involves helping social care and health care providers to identify and manage risks, responding to concerns about abuse or neglect, and offering guidance and support to those who need it most. You will work closely with a variety of professionals and partner agencies, always with the goal of promoting welfare and ensuring people receive safe, high-quality care. This is a supportive role at the heart of preventative work-building strong relationships with care providers, strengthening their safeguarding knowledge, and helping them deliver consistently good standards of care across the health and social care sector.

Responsibilities
  • Risk Assessment:
    Conduct thorough assessments to determine the level of risk faced by individuals and identify protective factors.
  • Investigations:
    Lead and participate in investigations of reported abuse, neglect, or exploitation cases.
  • Case Management:
    Develop and implement care plans that address the needs and risks identified during assessments. Monitor and review the effectiveness of these plans.
  • Advocacy:
    Act as an advocate for vulnerable individuals, ensuring their voices are heard and their rights are protected.
  • Multi-Agency

    Collaboration:

    Work closely with other professionals and agencies, such as the police, healthcare providers, and educational institutions, to coordinate care and support.
  • Record Keeping:
    Maintain accurate and detailed records of all assessments, plans, and actions taken to ensure accountability and continuity of care.
  • Training and Development:
    Stay updated on best practices in safeguarding through continuous professional development and training opportunities.
  • Community Engagement:
    Engage with the local community to raise awareness of safeguarding issues and promote preventive measures.
  • Crisis Intervention:
    Provide immediate support and intervention in crisis situations to ensure the safety and well-being of vulnerable individuals.
  • Legal Compliance:
    Ensure all actions and interventions comply with relevant legislation, policies, and procedures.

FACT operates countywide; therefore, access to a car with business insurance is essential.

Hospital Duty Shifts

You will be expected to work duty shifts within the hospital as part of your role.

Shift Schedule
  • 09:00-17:00 during community-based weeks.
  • 08:00-17:00 during hospital duty weeks.

There is an expectation to work weekends and bank holidays as part of the team rota.

You will be an empathetic and compassionate individual, who can support vulnerable individuals. Through your previous roles, you will have developed the ability to work under pressure, handling complex situations and solving problems. You'll need to have great communication skills, and work well in a team environment. It is also important that you have knowledge of relevant legislation and safeguarding practices for this role.

You may have a degree or NVQ 4, or the ability to demonstrate knowledge gained through detailed practice and application for procedures in adult social care.

This role does not meet the legal requirements for visa sponsorship under the Skilled Worker route. We cannot accept individuals on full-time sponsorship who are seeking this as a second job or as their main job, as care is no longer classified as a Skilled Worker occupation.
