Job responsibilitiesComprehensive Patient Assessments:
Conduct thorough health assessments to develop and
implement personalized care plans that meet each patients unique needs.
Vaccinations and Injections:
Administer immunisations, injections, and other treatments following best practice guidelines and ensuring patient comfort.
Research:
Potential opportunity to join our research team.
Chronic Disease Management:
Monitor and manage long-term conditions such as diabetes,
asthma, and hypertension, using evidence-based strategies to improve patient outcomes.
Expert Wound Care:
Provide skilled wound care management, including dressings, pressure
relief, and post-operative care to promote healing and recovery.
Health Promotion &
Education:
Actively engage in health promotion activities, offering education
and guidance on topics like smoking cessation, diet, and lifestyle changes to support long-term
Assist in Minor Surgery:
Support GPs and other clinicians during minor surgical procedures,
ensuring smooth preparation, execution, and post-procedure care.
Triage & Follow-Up:
Efficiently manage patient triage, assessing urgency, providing immediate
care, and coordinating follow-up for ongoing treatment or referrals.
Patient Records & Documentation:
Ensure that patient records are meticulously maintained,
documenting care plans, treatments, and progress with accuracy and attention to detail.
Collaborative Care:
Work closely with GPs, healthcare assistants, and allied health
professionals to deliver integrated, patient-focused care.
Quality Improvement & Innovation:
Play an active role in quality improvement initiatives, helping
to shape the development of services that continually enhance patient care.
Training & Development:
Participate in regular training programs to enhance your skills, stay
updated with the latest healthcare innovations, and ensure continued professional development.
Person Specification Qualifications- NMC registered
- Qualified in asthma care management
