West Northamptonshire Council is recruiting a Care Support Assistant to support individuals after discharge from hospital, helping them regain independence and daily living skills in a safe, supportive environment.
You will work within a multidisciplinary team to deliver personal care, mobility and rehabilitation, contributing to goal achievement and confidence building. Experience in health or reablement is advantageous, with NVQ Level 2 in Social Care (or equivalent) preferred.
