HCRG Care Group is recruiting an Advanced Clinical Practitioner for the Rapid Response Service covering the Swale locality, with bases at Sittingbourne Community Hospital and Sheppey Community Hospital. You will deliver autonomous advanced clinical assessment, diagnosis, treatment and care planning for urgent needs, collaborating with GPs, nurses and other partners.
The role requires independent prescribing, NMC registration and a strong background in urgent or community care, with 37.5 hours
