Weare seeking an enthusiastic, NMC-registered Nursing Associate to join ourexpanding primary care team. You will work collaboratively within ourmulti-disciplinary environment to deliver high-quality nursing care that supports the health and wellbeing of our local community of 12900+ patients.
This is an excellent opportunity to develop your skills in general practice within a supportive and inclusive environment.
Thisrole requires flexibility to work across different clinical areas within the practice, adapting to varying patient needs and service demands. You will workboth independently within your scope of practice and collaboratively as part ofthe wider healthcare team.
Provide holistic, person-centred care to individuals, families, and communities, including health assessments, wound care, and administering medication as delegated.
Chronic disease management clinics for diabetes,asthma, COPD and hypertension. Conduct comprehensive NHS health assessments, smokingcessation, weight management, Learning Disability annual assessments and lifestyle counselling. Provide health promotion,vaccination services including seasonal flu, COVID-19, and travel immunisations and preventative care screening including cervical smears, bowel screening. Perform clinicalobservations, wound care, and minor surgery procedures. Support clinical governance,safeguarding, and infection control protocols. Collaborate with GPs, Practice Nurses, and wider healthcare teams.
Mentor healthcare assistants and support practicedevelopment initiatives.
Maintain accurate, timely, and confidential patient records usingelectronic health systems, ensuring compliance with data protection and information governance policies.
Abington Park Surgery is a Northampton town centre practice with a patient list of 12900 patients and growing. We are a 5 Partner practice supported by a very cohesive clinical nursing, reception and admin teams. We are part of the Grand Union Primary care network and under the governance of Northampton ICB. We are a GP training practice , providing training not only to GP 's but also medical school students and nurses.

Chronic Disease Management
Learning Disability health checks including annual health checks and health action plans
Medication management including reviews,reconciliation, and monitoring
Patient education and self-management support for chronic conditions
Care planning and goal setting with patients and familiesHealth Promotion and PreventionNHS Health Checks and cardiovascular risk assessments
Health promotion activities including smoking cessation, weight management,and lifestyle advice
Vaccination services including seasonal flu, COVID-19, and travelimmunizations
Screening programs including cervical screening, bowel screening coordination
Preventative care and early intervention strategiesClinical Assessment and TreatmentComprehensive nursing assessments using systematic approaches
Clinical observations including vital signs, ECGs, and physiologicalmonitoring
Wound care management including assessment, treatment, and healing monitoring
Minor illness management within scope of practice and competency
Clinical procedures as trained and assessed competentCollaborative Care DeliveryMulti-disciplinary team working with GPs, Practice Nurses, and allied health professionals
Care coordination and case management for complex patients
Referral management to secondary care and community services
Liaison with district nurses, social services, and external agencies
Professional and Governance Responsibilities Maintain NMC registration and meet revalidation requirements
Adhere to clinical protocols and evidence-based practice guidelines
Participate in clinical audits and quality improvement initiatives
