Are you a compassionate and motivated Registered Nurse looking to develop your skills in a specialist community service? We are seeking a Band 5 Community Wound Care Nurse to join our dedicated Community Wound Service, supporting patients with complex and hard-to-heal wounds across Northamptonshire.
This is an exciting opportunity to become part of an Outstanding CQC-rated Trust, committed to delivering safe, high-quality and patient-centred care. You'll work closely with the Tissue Viability Team, senior nurses, and colleagues across our community nursing service to provide expert assessment, evidence-based interventions and personalised care plans.
Responsibilities- Deliver complex wound care management including leg ulcer treatment and wound assessment.
- Perform and interpret Doppler assessments to support effective treatment planning.
- Provide wound care in clinic settings, patients' homes and care environments across the county.
- Educate and empower patients, families and carers to promote self-care and better outcomes.
- Maintain accurate, confidential clinical records in line with NMC and Trust standards.
- Work collaboratively within multidisciplinary teams to ensure safe, effective patient care.
- Participate in audits, service improvement and ongoing professional development.
This role requires independent travel across Northamptonshire and the ability to carry equipment and undertake wound care in both clinic and home settings.
About you- Current NMC registration (Adult Nurse).
- Experience with in community nursing and managing hard-to-heal wounds.
- Skills in leg ulcer management and Doppler assessment.
- Understanding of tissue viability principles and wound care best practice.
- Excellent communication, organisational and time-management skills.
- Ability to work autonomously and as part of a multidisciplinary team.
- A full UK driving licence and access to a vehicle.

Person Specification Registration and Qualifications Essential- NMC registered Adult Nurse with post-Registration community Nursing experience.
- Accredited university course in Wound Treatment or Tissue Viability obtained in the last 2 years.
- Accredited university course in Wound Treatment or Tissue Viability obtained over 2 years ago.
- Experience working within a Community Nursing or Primary Care as a Registered Nurse.
- Current leg ulcer assessment competency (including Doppler assessment).
- Experience using System One Patient Electronic Record.
- Demonstrate experience in vacuum-assisted closure dressings.
- Demonstrate experience in communicating with the wider MDT.
- UK car driver and owner.
