West Northamptonshire Council is seeking an Adult Social Care Worker to help adults live independently, safely and with access to the right support. You will assess needs, plan and coordinate services, and work with families, carers and partner organisations to secure positive outcomes.
In this role you will join a supportive Adult Services team across diverse settings, across community services, hospital discharge and transition services. DBS checks apply and safeguarding is a priority.
