Position: Flexible Community Nurse Position Ontario

Key Responsibilities:

• Develop individual Care Plans for patients

• Provide in-home and virtual nursing services

• Collaborate with multidisciplinary care teams

• Communicate effectively with patients and families

• Carry out other clinical responsibilities as necessary

Requirements:

• Registered Nurse or Registered Practical Nurse qualification

• Active registration with the College of Nurses of Ontario

• Strong passion for patient-centered care

• Valid driver’s license for travel

• Maintain an up-to-date immunization record
