Registered Nurse (RN) in a community nursing role. Responsible for scheduled visits to care for patients in various settings, helping patients remain comfortable in their homes for as long as possible.

• Use appropriate assessment tools/techniques to evaluate patients’ individualized needs
• Execute and continuously monitor care plans

• Maintain and update professional patient records related to encounters and plans

• Registration with the CNO as a Registered Nurse in good standing
• Valid driver’s license and access to a vehicle
• Experience in delivering community care (asset)

• Required on‑call coverage, including after‑hours, to ensure patient safety and meet patient care needs

Full‑time. Hourly salary range: $30.83 – $36.54.
