Job Description   Sign on Bonus: $10,000
Registered Nurse (RN) in a community nursing role. Responsible for scheduled visits to care for patients in various settings, helping patients remain comfortable in their homes for as long as possible.

What You’ll Do   Use appropriate assessment tools/techniques to evaluate patients’ individualized needs
Execute and continuously monitor care plans
Engage patients collaboratively, building rapport and trust
Maintain and update professional patient records related to encounters and plans
What You Bring   Registration with the CNO as a Registered Nurse in good standing
Valid driver’s license and access to a vehicle
Experience in delivering community care (asset)
Reliability and flexibility
Willingness to travel to and from patient residences within a designated area
Passion for making a difference in your community
Required on‑call coverage, including after‑hours, to ensure patient safety and meet patient care needs
Compensation  Full‑time. Hourly salary range: $30.83 – $36.54.
