Key Responsibilities:

• Create and manage personalized Care Plans

• Provide nursing services both in-person and online

• Communicate with the interdisciplinary team and families

• Collaborate on various clinical duties as necessary

• Engage in community-focused health initiatives

Requirements:

• RN or RPN qualification

• Valid CNO registration

• Strong interpersonal skills and a desire to help

• A valid driver's license for travel needs

• CPR Certification and immunizations up to date
