You will also navigate system challenges and advocate for at‑risk patients in need of navigation services.
Key Responsibilities- Address obstacles affecting patient flow from admission to discharge
- Coordinate plans of care with the multidisciplinary team
- Manage system navigation and bed placement for optimal patient care
- Raise awareness of patient navigator roles within the healthcare team
- Identify patients requiring specialized navigation services
- Must be Registered with the College of Nurses of Ontario
- Minimum two years clinical experience in patient flow
- Knowledge of various bed types and admission criteria
- Familiarity with patient flow tools and community resources
- Robust communication and leadership skills
