Key Responsibilities:

• Tackle challenges affecting patient flow from admission to discharge

• Coordinate care plans with the multidisciplinary teams

• Manage navigation system and optimize bed placements

• Promote awareness of patient navigator roles

• Identify patients who require specialized navigation

Requirements:

• Registration with the College of Nurses of Ontario

• Two years of clinical experience in patient flow

• Knowledge of bed types and admission criteria

• Familiarity with community resources and flow tools

• Ability to communicate effectively and lead teams
