Key Responsibilities:
• Conduct detailed assessments and consultations for patients
• Partner in developing and evaluating clinical practices
• Liaise with local hospitals for patient care
• Manage controlled substance prescriptions
• Order diagnostic tests and analyze results
Requirements:
• Valid Nurse Practitioner Registration in Ontario
• Previous experience in geriatrics and chronic care
• Up-to-date CPR Certification
• Clear Vulnerable Sector Check
• Bilingual applicants preferred
