Take on a vital role as a Nurse Practitioner in our Specialized Geriatric Clinic, focusing on improving the quality of life for older adults. Collaborate with a committed health team to deliver optimal care solutions.
This role requires a Nurse Practitioner with extensive experience and a strong grasp of geriatric health needs. With a minimum of five years in a healthcare setting, candidates must hold an RN (EC) license and a Geriatric Assessor Certification. You'll play a key role in assessing patients, developing treatment plans, and navigating complex healthcare systems.
Key Responsibilities:
• Drive comprehensive assessments and collaborative care
• Coordinate care plans and manage diverse geriatric needs
• Engage in continuous quality improvement and best practices
• Manage prescribing practices within the scope of care
• Communicate effectively with patients and families
Requirements:
• Registration with the College of Nurses of Ontario as RN (EC)
• Minimum five years working with older adults required
• Previous clinical experience in mental health preferred
• Current BLS certification for Healthcare Providers
• Must possess advanced English/French language skills
