Position: Nurse Practitioner for Geriatric Care
Enhance the health of older adults as a Nurse Practitioner with the Specialized Geriatric Clinic in North Bay. Collaborate on care plans and optimize functional independence while coordinating complex needs.

This position requires strong collaboration with patients, care partners, and a specialized health team. The Nurse Practitioner will facilitate comprehensive assessments and provide consultations, ensuring exceptional well-being for older adults. A commitment to evidence-based practices and experience in geriatric care is essential for this role.

Key Responsibilities:

• Develop collaborative care and treatment plans

• Conduct comprehensive geriatric assessments

• Coordinate complex care needs for older adults

• Facilitate consultations with care partners

• Optimize patients' functional independence

Requirements:

• Current RN (EC) registration with CNO

• Minimum 5 years of geriatric care experience

• BLS certification within the past year

• Geriatric Assessor Certification required

• Bilingual in English and French preferred
