Foundation Health is seeking Registered Nurses for agency staffing support in home care services. This role requires assessing patient needs, collaborating with healthcare teams, and advocating for patients' rights and dignity. Critical thinking and experience with diverse patient needs, including dementia care, are essential for this position.
Key Responsibilities:
• Collaborate with patients and families on care plans
• Evaluate and adjust treatment plans in consultation
• Advocate for patient rights and dignity
• Use clinical judgment to respond to patient changes
• Manage complex family dynamics during care
Requirements:
• Registered Nurse in good standing with College of Nurses of Ontario
• Physical ability to assist clients and lift 50lbs
• Valid CPR and First Aid certifications
• Completed vulnerable sector check
• G2/G driver's license and reliable vehicle
