As a GEM Nurse, you will leverage your geriatric expertise while working with the Clinical Geriatric Services Team. You'll implement comprehensive assessments and advocate best practices for elderly care, engaging with patients and families. This role values leadership, program development, and a dedication to quality improvement in geriatric services.
Key Responsibilities:
• Implement comprehensive geriatric assessments
• Advocate best practices for elderly care
• Collaborate with multidisciplinary healthcare teams
• Provide health teaching to individuals and groups
• Manage and evaluate programs for the elderly
Requirements:
• Current registration with Ontario's nursing college
• Minimum 2 years of experience in elderly care
• Completion of RGPc Certificate Program
• Bilingual in English and French strongly preferred
• Demonstrated leadership skills in managing geriatric services
