You will play a critical role within the North East Behavioural Supports Ontario and Seniors’ Mental Health programs, working collaboratively to promote wellness and prevent hospitalization among older adults. This position requires comprehensive mental health assessments, transitional support, and treatment recommendations based on best practice principles. Your clinical expertise will guide care partners in addressing challenging responsive behaviors and optimizing patient outcomes across the Timiskaming District.
Key Responsibilities:
• Conduct geriatric mental health and behavioral assessments
• Provide specialized recommendations to care partners
• Collaborate with diverse healthcare sectors
• Offer outreach services to enhance patient independence
• Maintain a focus on quality of life for clients
Requirements:
• Current registration with the College of Nurses of Ontario
• Minimum of three years in applied clinical nursing
• Certifications in Trauma-Informed Care and Basic Life Support
• Advanced knowledge of psychotropic medications & care transitions
• Valid driver’s license for travel
