The Full-Time Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies. The RN coordinates the total plan of care with other healthcare professionals involved in care, ensuring continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources.

All services are provided within a Point of Care setting.
