The Relief position offers flexibility and the opportunity to make a difference while working with adults experiencing intellectual disabilities, mental health challenges, and physical limitations. Our Day Habilitation programs serve adults aged 23-85, focusing on individualized service plans, daily curriculum, and community integration.

As a Per Diem Day Hab Support Staff you will provide direct care, support a structured environment, and advocate for individuals served within the Day Program.

• Assist in developing individualized, person-centered treatment plans and play a lead role in the daily implementation plans.
• Assist individuals to access and/or provide transportation.
• Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
• Review and complete all required documentation.

• High School Diploma, GED or equivalent, required.
• Valid Driver's License and acceptable driving record, required.
• Reliability, willingness to learn, and openness to new opportunities.
