An essential member of the multidisciplinary clinic team, the Nurse Practitioner/Physician Assistant provides comprehensive outpatient care to individuals diagnosed with cancer, from diagnosis through treatment during the acute phase of survivorship. This position is responsible for coordinating care and ensuring appropriate referrals to ancillary and supportive resources to address physical, psychosocial, and other identified needs.
Will see patients at two or three of our clinics in North County, West County, South County, and our Center for Advanced Medicine located in downtown St. Louis.
Job Description PrimaryDuties & Responsibilities
- Evaluates survivors’ needs (physical, psychological, and social) using assessment tools such as distress screening to connect patients to referrals and support services, such as dietitians, social work, and physical therapy.
- Provides psychosocial support and facilitates referrals to psychology services as needed.
- Adheres to NCCN survivorship clinical guidelines and other evidence‑based practices.
- Provides direct patient care to include obtaining patient history, performing patient assessment via observation, interview and examination; orders, performs and interprets diagnostic studies; performs preventative health assessments, screenings, immunizations and care; provides direct treatment and management of acute and chronic disease conditions; develops and implements patient‑centered plan of care in collaboration with physicians, patients, consultants, nurses, ancillary care providers and others as appropriate to the patient's condition.
- Provides management services for clinical research study projects; monitors participating subjects, including medication administration and family/patient education.
- Works in accordance with established departmental policies and procedures, objectives, quality assurance programs, and safety, environmental and infection control standards.
- Regularly participates in constant quality improvement and process improvement activities centered on the goal of providing exceptional, evidence‑based patient care.
- Enhances professional growth and development through educational programs, current literature, in‑service meetings and workshops.
- Contributes to the advancement of the rehabilitation nursing profession as a whole through research, publications, and presentations.
- Bills for professional services as permitted by the collaborative practice agreement.
- Interprets diagnostic test results for deviations from normal.
- Performs other duties as assigned.
