PHYSICAL THERAPIST ASSISTANT – MALLEY TRANSITIONAL CARE – NORTHGLENN, CO PRN

Malley Transitional Care is a skilled nursing facility located in North Glenn, CO offering vent/trach services, long‑term care, post acute and outpatient services.

• Utilize evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment interventions to patients with physical functioning disorders under PT supervision.
• Administer various procedures as part of the rehabilitation plan including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices.
• Assess patient progress and communicate with supervising PT to adjust treatments accordingly, documenting in the electronic system per regulatory requirements.
• Consult and counsel patients, families, caregivers and other service providers related to physical disorders.
• Provide input to supervisor for discharge summaries per regulatory and clinical requirements.
• Document all regulatory and clinically required information and maintain patient records according to company policy.

• Licensed Physical Therapist Assistant.
• Prior experience in a skilled nursing setting is preferred; new graduates are welcome.

Pay Range: $25 - $36 per hour (PRN).
