– Job Duties
- Encourage and guide individuals to meet their goals following person-centered and behavioral support/care plans.
- Transport individuals to appointments, advocate on their behalf, and assist with documentation and communication.
- Monitor healthcare routines and report and irregularities immediately to the Executive Director and Regional Clinical Care Coordinator.
- Closely monitor residents in poor health or at risk for complications, following care plans.
- Assess residents in the hospital and consult with leadership to determine appropriateness for return.
- Manage Med Techs, Care Aides, and Direct Support Provider (DSPs); interview, hire, orient, coach, counsel, and evaluate staff with guidance from the RD.
- Provide functional supervision, assign work, and verify accurate completion.
- Oversee care instruction and training during new hire orientation.
- Serve as primary scheduler for the nursing department with assistance from the House Manager.
- Assist with resident assessments, admissions, and discharges.
- Ensure resident support plans, charts, and treatment schedules are followed by staff.
- Observe and report any signs of abuse or neglect; take immediate action to prevent harm and follow company reporting policies.
- Establishes and maintains a positive, respectful, and supportive environment for all residents, co-workers, families, and facility guests.
- Assures a welcoming process occurs for all new residents and co-workers.
- Works in partnership with the Executive Director, DOW, Admissions Coordinator to place them appropriately within the continuum levels of care levels.
- Implementation and training in Electronic Health Record system.
- Participate in an on-call rotation as scheduled.
- Primary scheduler for the nursing department.
- Performs other duties as assigned.
– Job Duties
- Attend and participate in staff meetings, projects, special events, committees, and mandatory training as appropriate.
- Maintains safety and health rules by participating in training along with implementing and following protocols related to OSHA & CDC regulations and Cardinal Senior Management Standards as required.
- Promotes and encourages individualized resident independence while maintaining resident dignity, individuality, confidentiality and following infection control policies.
- Works as a member of a team to provide essential nursing services and uses effective communication skills.
- High School Diploma or equivalent.
- LPN, Med Tech, or equivalent work experience.
- Experience with Microsoft Office.
- Ability to read, write, speak and understand the English language.
- Ability to walk continuously, bend frequently, and sit occasionally.
- Ability to complete and maintain documentation regarding a resident’s status.
- Ability to operate computer systems and phone systems.

- Specialized training through Muskegon County CMH, meeting the requirements of Specialized Residential Licensing and LRE (Lakeshore Regional Entity) standards.
- CPR and First Aid Certification.
- Mandt De-Escalation Training.
- Medication Administration Certification.
