We serve facilities across the United States with a variety of services to improve the quality of care delivered, refine facility processes, and enhance critical relationships, including multispecialty physician services, remote patient monitoring, and more.

Shift

Structure
  • In-person rounding shifts start around 7 AM local time with within‑reason flexibility
  • Shifts are typically available Saturday and Sunday and on major holidays
  • Weekend coverage and two holidays per calendar year
  • Charting/documentation can be completed off‑site
  • This is an in‑person position
Essential Functions and Responsibilities
  • Provider is responsible for rounding and managing the clinical care of the population at the assigned SNF
  • Rounding daily and prescribing medications and treatments for residents at SNFs
  • Provide high‑quality, patient‑centered, comprehensive primary care services to patients residing in SNFs
  • Timely documentation of all patient care/encounters
  • Availability for phone/secure messaging consultations with facility nursing staff
  • Documentation completed in Theoria’s proprietary electronic health record (EHR) system, Chart Easy™
  • Common facility EHR systems include Point Click Care , Matrix Care, and American Health Tech
Physical Requirements
  • Move frequently and briskly inside a facility to see patients, meet with staff, and attend to patient needs
  • Punctual in‑person attendance, typically between 7 AM and 1 PM for rounders
  • Continuous operation of app‑based EMR/notes system via computer or mobile device (iPad, phone, etc.)
  • Ascend and descend stairs in a facility
  • Ability to position oneself to obtain records, tools, or equipment located below the knee or above the shoulder
  • Effective communication with patients, managers, and facility staff
  • Observe details in close range to provide accurate medical recommendations and timely updates to managers
  • Lift or move at least 25 pounds
Requirements and Qualifications
  • Valid PA/NP license in the state of practice
  • Valid NP/PA degree from an accredited institution
  • Active DEA license (new graduates may receive assistance from Theoria)
  • General post‑acute/SNF knowledge or experience preferred
  • Familiarity with EMR systems and charting practices
  • wRVU productivity
  • Malpractice insurance covered by employer
  • Opportunities for training (e.g., wound care)
  • Equipment is not provided
  • All practice expenses covered
