Position Summary: Under the direct supervision of a licensed nurse, performs individually assigned care activities for the chronically ill and technologically dependent residents based on established resident care policies and procedures. Participates in resident care as an active member of the healthcare delivery team. Specific areas of responsibility include monitoring of resident vital signs, providing resident hygiene and comfort measures, assisting with medical procedures and fulfilling nutritional and grooming needs.
Provides general direct nursing care to assigned residents. Responsible to:
Registered Nurse, Licensed Vocational Nurse, Clinical Manager.
- Licensure/Certification:
Current California Certified Nursing Assistant Certification, Current BCLS certification - Experience:
No prior C.N.A. Experience required. New grads welcome! - Knowledge of procedures and techniques involved in administering simple treatments and providing related bedside patient care services, as permitted by state regulation.
- Knowledge of basic medical asepsis and standard precautions.
- Willing to work rotating shifts and different units.
- Ability to contribute to a patient-centered environment.

Skills:
Must be able to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manual. Must be easily understood through verbal communication in the English language. - Mathematical
Skills:
Must be able to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
- Interacts with customers, families, visitors, center and Reliant subsidiary personnel.
- Carries out other tasks as requested in situations where hands‑on intervention/participation may be required.
- Successful completion of a state approved certified nursing assistant program.
- Minimum high school education or high school equivalency diploma is preferred.
- Must be able to read, write, speak and understand spoken English at the work site.
- Must provide verification of TST as required by state law.
- Basic knowledge of computer use or willingness to learn.
- Must attend a minimum of 12 hours continuing education programs.
- Provides patient care in a manner conducive to safety and comfort. Assists patient with ADL, obtains vital signs, assists with ambulation, positions patients, applies adaptive equipment, cares for personal belongings, assists physician/nurse with treatments, makes beds, accompanies patients to appointments, ensures patients are ready for rehab, notifies nurse if , participates in care planning, performs skin care treatments, feeds/hydration, records intake/output, uses EMR, answers call, cleans areas, applies standard precautions, participates in orientation of new CNAs, reports changes, collects and handles linen, cleans equipment, transfers belongings, assists post‑mortem care, assists with discharge, promotes safety culture, coordinates with departments, maintains PHI, works beyond shift if needed, performs other duties.
