since 1971, hyc has eliminated barriers to self-sufficiency by empowering southeast los angeles and long beach youth and families.

the mission of helpline youth counseling (hyc) is to help build individual, family, and community health, well-being, prosperity, and equity. Hyc serves clients and community members with trauma-informed, strength-based, person-centered prevention, early intervention, education, and treatment services.

this position is for a collaborative grant project called thriving together between helpline youth counseling (hyc) agency and academic partner azusa pacific university (apu) funded by the california community foundation (ccf).

project thriving together seeks to support service expansion and capacity building of behavioral health community entities that serve young adult populations with co-occurring substance use and mental health complexities in under-resourced areas of los angeles county.

hyc has an immediate opening for a full-time thriving together lead case manager who will be responsible for providing care to participants that promotes resilience and support, lifestyle changes, member skill development and self-management.

responsibilities
  • provides specialized case management to ensure their co-occurring mental, social and substance use needs are adequately identified and addressed through skill development/management and community linkages-connections.
  • assist with increasing local level outreach to community locations in southeast los angeles county serving and supporting the young adult population, including drop-in mental health centers, temporary housing shelters, continuation schools, community colleges, adult schools, law enforcement settings, and service sites including access centers.
  • provides culturally and linguistically appropriate communication and information to engage clients.
  • the lcm will coordinate mental health/sud care services, provide mental health/sud education, and facilitate care in a timely manner.
  • maintain accurate and timely documentation of all job-related activities.
  • coordinate with external care providers and entities providing the clients with some level of care.
  • will be required to participate in required training and clinical supervision.
  • must have excellent organizational skills and must complete all required reports and documentation in a timely manner.
  • participate in weekly clinical supervision or case conferences that may include other members of interdisciplinary team.
  • performs other duties as assigned by supervisor.
requirements
  • bachelor’s degree in the behavioral sciences.
  • one (1) year’s full-time work experience in community-based services field.
  • permanent u.s. Residency or citizenship.
  • ability to pass criminal clearance background check.
  • ability to successfully pass a tb screening test.
  • proof of u.s. Citizenship or permanent residence.
  • willingness to work a flexible schedule, including evenings.
  • current auto insurance, and a good driving record.
  • valid california state driver’s license.
  • ability to manage multiple tasks, projects and priorities.
  • excellent administrative and verbal skills.
preferred qualifications
  • experience in working within the southeast region of los angeles county and a familiarity with the population of the area.
  • experience in networking with school and community agencies in providing client care services programs.
  • experience in working in a community-based organization.
  • excellent verbal and written communications skills preferred in both english and spanish.
