Base Pay Range$70,000.00/yr – $85,000.00/yr

- Assist in the collection of resident assessment data (MDS), in accordance with current rules, regulations, and guidelines, that govern the resident assessment, including the implementation of CAA and triggers.
- Review updates of written policies and procedures that govern the development, use and implementation of the resident assessment (MDS) and care plan.
- Participate in an ongoing quality assurance program for the collection of resident data and resident care plans.
- Participate in functions involving discharge plans as may be necessary.
- Implement procedures with the RN resident assessment/care plan coordinator to inform all assessment team members of the arrival of newly admitted patients.
- Develop the schedule of activities required for completion of the RAI process.
- Assist with coordinating the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
- Conduct daily, concurrent utilization review via chart audit to ensure that change of condition RAI’s are initiated when appropriate.
- Conduct day concurrent utilization review via chart audits to ensure that discharge planning proceeds smoothly relative to resident’s progress and Medicare coverage status.
- Main the confidentiality of all resident care information.
- Previous experience as an MDS Coordinator preferred.
- Licensed Practical Nurse (LPN) or Licensed Registered Nurse (RN) in the State of Connecticut required.
- Must be an LPN or RN.
