Base Pay Range

$70,000.00/yr – $85,000.00/yr

Administrative Functions
  • Assist in the collection of resident assessment data (MDS), in accordance with current rules, regulations, and guidelines, that govern the resident assessment, including the implementation of CAA and triggers.
  • Review updates of written policies and procedures that govern the development, use and implementation of the resident assessment (MDS) and care plan.
  • Participate in an ongoing quality assurance program for the collection of resident data and resident care plans.
  • Participate in functions involving discharge plans as may be necessary.
  • Implement procedures with the RN resident assessment/care plan coordinator to inform all assessment team members of the arrival of newly admitted patients.
  • Develop the schedule of activities required for completion of the RAI process.
  • Assist with coordinating the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
  • Conduct daily, concurrent utilization review via chart audit to ensure that change of condition RAI’s are initiated when appropriate.
  • Conduct day concurrent utilization review via chart audits to ensure that discharge planning proceeds smoothly relative to resident’s progress and Medicare coverage status.
  • Main the confidentiality of all resident care information.
Qualifications & Experience Requirements
  • Previous experience as an MDS Coordinator preferred.
  • Licensed Practical Nurse (LPN) or Licensed Registered Nurse (RN) in the State of Connecticut required.
  • Must be an LPN or RN.
