Job Description & How to Apply Below

Mid-Fairfield Community Care Center is seeking a Bilingual Spanish/English Care Coordinator to join our Child First program, an evidence-based early childhood mental health model serving young children and their caregivers.

The Care Coordinator partners with a Master’s-level Clinician as a two-person Child First team to provide relationship-based, home- and community-based services to families with children prenatal through age 5. This role combines care coordination, family support, advocacy, and caregiver skill-building to help families address barriers, strengthen family functioning, and support healthy child development.

This is an excellent opportunity for someone with experience working with young children and families who is interested in developing specialized expertise in early childhood mental health. Child First provides extensive training, ongoing professional development, and regular reflective supervision.

What You’ll Do
  • Partner with a Child First Clinician to assess family strengths, needs, priorities, and challenges and develop an individualized Child and Family Plan of Care.
  • Provide hands-on care coordination, including resource navigation, advocacy, problem-solving, and coordination with community providers and family-serving systems.
  • Help families address needs related to housing, food, health care, benefits, child care, education, legal services, and other community supports.
  • Support caregivers in strengthening skills such as planning, organization, problem-solving, emotional regulation, and follow-through.
  • Use a reflective, relationship-based approach to engage caregivers and understand family strengths, goals, stressors, and barriers.
  • Support positive caregiver-child interactions and developmentally appropriate play during family visits.
  • Collaborate with early childhood programs, schools, health care providers, behavioral health providers, and other community partners.
  • Participate in Child First training and regular individual, team, and group reflective supervision.
  • Complete required documentation, assessments, data entry, and service tracking.
Qualifications

Required:

  • Bachelor’s degree or higher in social work, human services, psychology, child development, family studies, education, community health, nursing, or a related field.
  • Verbal and written fluency in Spanish and English.
  • Experience working with young children and/or families.
  • Ability to build collaborative, respectful relationships with families from diverse backgrounds.
  • Interest in early childhood development, caregiver-child relationships, family support, and community-based services.
  • Capacity for self-reflection and willingness to participate in reflective supervision.
  • Ability to provide services in family homes and community settings throughout the greater Norwalk area.
  • Availability to work two evenings per week, with appointments beginning as late as 6:00 p.m.

Preferred:

  • Two or more years of experience working with families with young children, particularly children birth to age 5.
  • Experience in care coordination, case management, family support, home visiting, early childhood services, or community-based services.
  • Familiarity with behavioral health, trauma-informed care, early childhood mental health, and/or community resources.

This is a full-time, 40-hour-per-week position , Monday through Friday, with flexibility based on family and program needs. Two evenings per week are required. Services are provided in family homes, community settings, and at Mid-Fairfield Community Care Center in Norwalk.

Salary $47,000-$52,000 per year based on experience, credentials, and skills. Employees may also be eligible for additional compensation for exceeding established quarterly productivity expectations.
