South Shore Health System is seeking a Clinical Social Worker to support SSH & SSMC clients, coordinating with the RN Case Manager to plan transitional care and secure services for quality outcomes. The role is on-site 4–5 days a week with no nights, weekends, or on-call hours.
You will work with a multidisciplinary team to optimize care coordination, monitor admissions and discharges, and document interventions clearly to support safe community transitions.
