Requirements:

Master's or Doctorate degree in Physical Therapy from an accredited college.
Current Physical Therapy license in Connecticut or ability to obtain one quickly.

Responsibilities:

The Physical Therapist / PT is responsible for assessing referred patients requiring rehabilitation services, including providing direct patient care to evaluate their medical condition, functional capabilities, limitations, restrictions, and rehabilitation potential.

Establish and administer a treatment program with specific goals based on the patient’s capacity and tolerance, under physician supervision.

Set realistic and achievable goals, document patient conditions, and educate patients and families on appropriate physical therapy methods.

Develop a plan of care for each patient.

Provide skilled physical therapy services in accordance with physician orders.

Assess patient needs, plan, evaluate, and modify care to meet therapy goals.

Collaborate with multidisciplinary teams to plan and evaluate patient care goals.
