Job Summary:

The Family Resource Coordinator works under the supervision of the Director of Family Bridge‑Eastern CT. This community‑based position involves working alongside Home Visitors to evaluate participants’ needs, referring clients to community resources, coordinating appointments, assisting with enrollments in diverse community resources, and helping families communicate with medical offices, social service agencies, DCF, and other organizations. All work with families is short‑term with the objective of increasing their ability to self‑advocate.

• Associate's degree and five years of experience working with clients in a health or social services setting, or any combination of education and experience that can meet essential tasks.
• Must obtain a Community Health Worker Certification within six (6) months of hire.
• Excellent computer skills (Microsoft Office Suite, Excel, Word, and Publisher).
• Strong communication skills.
• Valid driver’s license and daily access to a reliable and insured motor vehicle.
• Bilingual strongly preferred.
