Job Title

Sub-Acute Residential 6737

Overview

Full-time, 40 hours per week.

Schedule:

Monday, Wednesday 12 PM-8 PM, Tuesday, Thursday, Friday 9 AM-5 PM. Program:
Sub-Acute Crisis Stabilization (SACS) in Norwich.

Responsibilities
  • Participate as member of multi-disciplinary treatment team.
  • Track and coordinate client intake, discharge, services and referrals.
  • Assist with treatment planning and daily triage.
  • Educate and link individuals to self-help groups and natural support resources.
  • Assist with linking clients to community support systems, agencies and obtaining benefits.
  • Maintain documentation of contacts and communication to maintain quality services.
  • Work as liaison between agency and other relevant providers to coordinate care.
  • Assist with scheduling and providing transportation to and from appointments.
  • Participate in staff meetings, supervision, training and other meetings.
  • Provide crisis intervention and support as clinically indicated.
  • Additional duties as required.
Qualifications
  • Bachelor’s degree in human service-related field required.
  • Minimum of two years related experience preferred.
  • Valid driver’s license required.
  • Complete program specific training required; complete annual agency trainings.
  • CPR/First-Aid Certification may be required within 6 months of hire.

Compensation

$20.00/hr minimum; commensurate upon experience.
