Job Title
Care Coordinator:
Sub-Acute Residential 6737
Full-time, 40 hours per week.
Schedule:
Monday, Wednesday 12 PM-8 PM, Tuesday, Thursday, Friday 9 AM-5 PM. Program:
Sub-Acute Crisis Stabilization (SACS) in Norwich.
- Participate as member of multi-disciplinary treatment team.
- Track and coordinate client intake, discharge, services and referrals.
- Assist with treatment planning and daily triage.
- Educate and link individuals to self-help groups and natural support resources.
- Assist with linking clients to community support systems, agencies and obtaining benefits.
- Maintain documentation of contacts and communication to maintain quality services.
- Work as liaison between agency and other relevant providers to coordinate care.
- Assist with scheduling and providing transportation to and from appointments.
- Participate in staff meetings, supervision, training and other meetings.
- Provide crisis intervention and support as clinically indicated.
- Additional duties as required.
- Bachelor’s degree in human service-related field required.
- Minimum of two years related experience preferred.
- Valid driver’s license required.
- Complete program specific training required; complete annual agency trainings.
- CPR/First-Aid Certification may be required within 6 months of hire.
